What Actually Happens
A decision that carries significant risk is initially approved under operational pressure. It's documented as a temporary exception with specific time boundaries and conditions. However, the decision is never formally escalated to appropriate governance forums, and the time-bound limitation is never enforced. The exception silently becomes permanent operational practice.
Later, when assurance activities occur, they validate the presence of controls around the decision not the appropriateness of the decision itself.
